What Is the GAD-7?
The Generalised Anxiety Disorder-7 (GAD-7) is a validated, self-administered screening and severity tool for generalised anxiety disorder (GAD). Developed by Spitzer, Kroenke, Williams and Löwe in 2006, it consists of 7 items each scored 0–3, giving a maximum score of 21. It is the most widely used anxiety screening tool in primary care globally and is endorsed by NICE, the NHS IAPT programme, and the APA.
A GAD-7 score ≥10 has a sensitivity of 89% and specificity of 82% for generalised anxiety disorder. It is also sensitive to panic disorder, social anxiety disorder, and PTSD, making it a useful broadband anxiety screening tool.

GAD-7 vs PHQ-9 — Using Both Together
GAD-7 and PHQ-9 are the standard pair in primary care mental health assessment. They share the same 4-point frequency scale and take under 5 minutes combined. NICE recommends using both in any patient presenting with possible depression or anxiety, as comorbidity is common — up to 50% of patients with depression have clinically significant anxiety and vice versa.
Both are used as outcome measures in the NHS IAPT programme, with scores recorded at every session to monitor treatment progress. A reduction of ≥4 points on GAD-7 is considered a reliable change; below 8 is considered recovery threshold in IAPT.
